New Patients

We welcome requests for registration from new patients living within our practice boundary. 

Simply request a 'New Patient Registration Form' and pick up a Practice Leaflet from the surgery.  After your application has been accepted, we will write to you to arrange a health screening check if required with one of our Nursing Team.

You will be registered with the practice rather than an individual GP and may see any of the doctors.  You are able to express a preference of doctor and we will endeavour to fulfil this request wherever possible.

 We will require the following information to enable us to process your request.

• Name
• Address
• Date of birth
• Name and address of previous doctor
• NHS number
• 2 forms of identification
  • Photo ID e.g. driving licence or passport
  • Proof of address e.g. utility bill or bank statement

We are able to accept new patients onto our list who live within our practice area.

 

Existing patients who move outside our practice area will need to register with a surgery local to their new home.
