Our FeesFees for Services and Reports (Jan2010)
It is not necessary to make an appointment with the doctor if you require a doctor to sign a form, complete a claim form or provide a certificate. Simply hand the form to a receptionist together with your payment who will pass your form to a doctor for completion. You are requested to phone the surgery after 10 working days to check if your form is ready for collection.
